Model Release for Minors

Parent or Guardian Image Release and GDPR agreement for minors (under 13 years)

I am the parent or guardian of the child named below and I have the legal authority to execute this release on behalf of the child.
In consideration of the child’s engagement and participation in this photoshoot, I give Gorjus Photography by Kayleigh Williams permission to take images of the child named below.
I also understand that Gorjus Photography by Kayleigh Williams is not responsible for any expense or liability incurred as a result of my/ my child’s participation in this photography, including any issues incurred as a result.

GDPR - I grant permission for Kayleigh Williams of Gorjus Photography permission to use any taken images of me/ my child in the following ways – state clearly which parts if any are not to be used in the following:
On their websites*
On Social Media*
For Promotional Purposes*
Competition Entries*
Product Samples for Display*
Image Consent*
By signing this form I am aware of my right to withdraw my consent at any time and that this form will be stored within Gorjus Photography and not passed on to any third party.

I waive any right to royalties or other compensation arising from or related to the use of the images as approved above. I release, discharge, and agree to hold harmless the Photographer and its employees, heirs or agents and assign from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking, processing, or publication of the Photographs.

This release shall be binding upon me, my agents and legal representatives. I confirm that I am at least 18 years of age, and that I have read the foregoing and fully understand the contents of this agreement with Gorjus photography by Kayleigh williams, and that I am competent to execute this agreement.

I confirm that I agree to the nature of the photoshoot as stated below, and to the payment of the below fee. I am happy with the standard of the photographers work and the manner in which images are presented after editing. I am fully aware and trust that the photographer will edit the images which she deems the most visually and technically suitable to attain my online gallery and or desired package, or the inclusive package as stated below.

I hereby consent to the foregoing agreement on my child’s behalf and fully understand that once in possession of said images neither I nor anyone else are permitted to edit these images or use them in any way other than for personal use this restriction of use includes, (but not by way of limitation) illustration, promotion, advertising and trade.
